Dear incoming Yale Student,
Health On Track, Yale’s platform for uploading required health information for all students, is now live!
Thank you for making time this summer to meet with a healthcare provider and prepare your required health forms, which you can upload any time between now and August 1, when they are due for all new students enrolling in the fall.
(If you enrolled in a program beginning during the summer or as part of the FSY program, your deadline is July 15; please follow the instructions you received separately, not the ones below.)
Here is how to proceed:
Step one:
Watch this brief video, which will introduce you to Health On Track, the platform you will use for uploading your materials and tracking your health requirements.
Step two:
Make sure you know how to scan and combine forms into stand-alone files, which you will upload. Follow these instructions for iPhone or Android, or you may use a scanner.
Step three:
Review your required forms and make sure they are fully completed and translated into English. They must include:
- The same legal name you submitted to Yale.
- Your name and date of birth on each page.
- A signature or stamp from a healthcare provider in the indicated spaces.
Step four:
Scan your completed forms using your computer or phone as outlined in step 2. Each of the following multi-page forms must be combined into its own stand-alone file.
- Health and Physical Form: pages 7-8 of packet
- Immunization Form: pages 9-11 of packet
- If you are submitting lab tests (to confirm immunity, or for TB screening), upload as a separate file.
- TB Risk Assessment Form: pages 12-13 of packet
- Authorization to Treat (only if under age 18): page 14 of packet
Step five:
Log into Health On Track using your Yale NetID and password.
- Follow all on-screen instructions.
- Complete all 4 consents. You only need to do this once.
- Upload your documents to satisfy each requirement (this will take approximately 15 minutes).
Once you have successfully uploaded all your documents, you may log out of Health On Track. You will get an email summary with the status of your submissions, typically within about 10 minutes.
Common Questions
What if I am missing a form or a requirement?
Submit your forms by August 1 even if you are missing some information. Any remaining requirements or forms can be submitted individually. We will remind you weekly until your requirements are completed.
I don’t have a healthcare provider to complete the forms.
This information is required by Yale, and it is your responsibility to complete it before arriving on campus. If you don’t have a primary care provider, consider contacting a CVS Minute Clinic or other walk-in center, or a Federally Qualified Health Center.
My healthcare provider gave me a printout of my immunizations but didn’t fill in the form with the dates.
If you have a printout from your health record, you can write the vaccination dates into the form yourself. Make sure you still have a provider’s signature or stamp.
I’d like to request an exemption from a requirement.
Go to the Contact Us tab in Health On Track and select request an exemption from the drop down. The exemption form must be completed and signed by a healthcare provider.
